Requests for Veterans Military Information Individuals may request copies of deceased veterans records by submitting a request in writing to the nearest VA regional office. Call 1-800-827-1000 for the address. Please provide as much information as possible to help the VA office to identify the requested records. Some examples: full name, VA file number, branch of service, service serial number/SSAN, exact date of birth or death, and entitlement or discharge dates. The VA does not maintain information by organization type or duty station. The information is subject to applicable laws and regulations.
Requests for your own record To request a copy of your own record, send a written request under the privacy act to the nearest VA regional office. Include as much identifying information (above) as you can and try to include your VA file number or VA claim number. If you have never filed a claim for VA benefits, you must submit a Standard Form 180, Request Pertaining to Military Records. The form is available from the nearest regional office. Mail the form to the appropriate office listed on the back of the form.

Be sure to include full name and preferably service number.
